When you fill out a form on our websites, you may provide your name and other contact information, including your company’s name, your e-mail address, phone number, and your mailing address or the mailing address of your company or other personal information. If you do not wish to fill out all of the information on the form, you are not required to do so. Most of our registration forms require only your name and e-mail address. When you submit information to us through Gotoeis.com, you will receive a confirmation e-mail if you enter a valid e-mail address.

    • SMS Program: When you opt in to our SMS program, you provide your phone number and consent to receive text messages. We may also collect additional information you provide through SMS interactions, such as responses to messages or inquiries.

  3. Use of Personal Data Collected. Through Gotoeis.com and our SMS Program, EIS uses the personal data information you provide to answer your question or resolve your problem. SMS Terms and Conditions


  1. Program Description: By signing up for our SMS program, you agree to receive recurring automated text messages, including:

    • Promotional Messages: Offers, discounts, and marketing campaigns related to our products or services.

    • Transactional Messages: Order confirmations, service updates, appointment reminders, and other service-related notifications.

    • Conversational Messages: Responses to your inquiries or customer support interactions.

  2. Message Frequency Message: frequency varies based on your interaction with us. You may receive up to 4 promotional messages per month, transactional messages as needed (e.g., per order or appointment), and conversational messages based on your inquiries. Additional messages may be sent periodically based on your engagement with our program.

  3. Message and Data Rates Message: and data rates may apply, depending on your mobile carrier plan. You are responsible for any fees charged by your wireless carrier for sending or receiving SMS or MMS messages. Please consult your carrier for details.

  5. Opt-Out Instructions: You may opt out of our SMS program at any time. To unsubscribe, reply STOP, END, CANCEL, UNSUBSCRIBE, or QUIT to any message you receive from us. You will receive a confirmation message verifying of your opt-out selection, and you will no longer receive messages from our program. Note that opting out is specific to the phone number and campaign you are enrolled in. To rejoin, you must opt in again through our standard process.

  6. Consent and Auto dialer Disclosure: By opting in to our SMS program, you consent to receive text messages sent via an automatic telephone dialing system (auto dialer) or other computer-assisted technology, as applicable. Consent to receive these messages is not a condition of any purchase.

  7. MMS Disclosure: If your mobile device does not support MMS messaging, our program will send SMS terminating messages (TMs) instead. We are not responsible for any issues arising from your device’s inability to receive MMS messages.

  8. Disclaimer of Warranty: Our SMS program is offered on an "as-is" basis and may not be available in all areas at all times. We are not responsible for any interruptions caused by product, software, coverage, or other changes made by your wireless carrier or other third parties.

  9. Dispute Resolution: Any disputes arising from your participation in our SMS program will be resolved through binding, individual-only arbitration, as detailed in the Privacy Policy section below or our general Terms and Conditions, if applicable.

General Provisions


  1. Governing Law: This combined document is governed by the laws of the State of Ohio, without regard to its conflict of law principles.

Dispute Resolution: Any disputes arising from this document, including the SMS Terms and Conditions or Privacy Policy, will be resolved through binding, individual-only arbitration under the rules of the American Arbitration Association. You waive the right to participate in class action lawsuit or class-wide arbitration.
